More pieces of information are filtering out from authorities in Los Angeles surrounding the accidental death of the late "Friends" star, Matthew Perry in late October. The actor was 54 years old. As RedState wrote at the time, Perry was found unresponsive in the jacuzzi of his home by an assistant, which TMZ first reported. It was the assistant who alerted authorities:

More details are surfacing about the circumstances surrounding Matthew Perry's death ... as sources tell us he actually died at his own house after some physical activity this morning.

We're told Matthew came home sometime in the AM after a 2-hour round of pickleball, and that he sent his assistant out on errand shortly thereafter. We're told when the assistant returned about 2 hours later, he discovered Matthew unresponsive ... and called 911.

A subsequent report by the LA Times filled in some details on the police response:

“Friends” star Mathew Perry was found dead Saturday in a hot tub at his Los Angeles home, law enforcement sources said.

Authorities responded about 4 p.m. to his home, where he was discovered unresponsive. The sources, who spoke on condition of anonymity because the investigation was ongoing, did not cite a cause of death. There was no sign of foul play, the sources added. A representative for Perry did not immediately respond to The Times’ request for comment.

Los Angeles Police Department’s robbery-homicide detectives are investigating the death. The cause of death will be determined at a later date by the Los Angeles county coroner’s office.

The Los Angeles County Medical Examiner has now released that report. Friday's autopsy report showed that Perry's cause of death was "the acute effects of ketamine," along with three other "contributing factors":

Matthew Perry died from the acute effects of ketamine, according to the autopsy report released Friday by the Los Angeles County Medical Examiner.[...]

The autopsy listed drowning, coronary artery disease and buprenorphine effects as contributing factors not related to the immediate cause of death. The manner of death was ruled an accident.

The report continued with an explanation of the presence of ketamine, a legal drug sometimes prescribed to treat depression:

Perry was on ketamine infusion therapy, with the most recent therapy 1 1/2 weeks before death, according to the autopsy report.

He had high levels of ketamine in his blood, and likely lapsed into unconsciousness and then went under the water, the autopsy report said.

He was reported to have been receiving ketamine infusions for depression and anxiety, according to the report, but the medical examiner writes the ketamine in his system at death could not have been from that infusion therapy because ketamine’s half life is 3 to 4 hours or less. His method of intake is listed as unknown.

I think some insights from one of Perry's former "Friends" castmates are worth including here. In the first interview she's done since the actor's death, which was published on Dec. 11, Jennifer Aniston told Variety that Perry was in a good place--and she had communicated with him the morning he died:

“He was happy. He was healthy. He had quit smoking. He was getting in shape. He was happy — that’s all I know,” she said, through tears. Next to her, Aniston’s “The Morning Show” co-star Reese Witherspoon held her hand as she wiped her tears. “I was literally texting with him that morning, funny Matty. He was not in pain. He wasn’t struggling. He was happy.”

She continued:

I want people to know he was really healthy, and getting healthy. He was on a pursuit. He worked so hard. He really was dealt a tough one. I miss him dearly. We all do. Boy, he made us laugh really hard.

Us, too, Jennifer. Rest in peace, Matthew.
